Integrity Home Health

Our Story

Integrity Home Health was opened by a husband and wife who immigrated to the US to start a business together where they could make a difference to patients in need of care. The company was acquired in 2018 by a business partnership looking for the right company that had a vision and purpose they shared. Integrity has proven to be a place where the family values it started on are in every part of the agency.

Integrity Home Health prioritizes the care of the patient and the experience of the workforce that makes that happen. Everyone plays their part in effective healthcare and it is our job to ensure that is the case every step of the way.

Services

Our services include skilled nursing, physical, and occupational therapy, speech therapy, medical social work, and home health aide. Our focus is on care continuity, timely support, and helping reduce re-admissions:

— Skilled Nursing —

 Ensure a seamless transition from hospital to home with our skilled nursing services. Our RNs and LPNs provide post-acute care, medication management, wound care, and chronic condition monitoring—helping reduce readmissions and support recovery at home. 

— Physical & Occupational Therapy —

 Our licensed therapists provide in-home rehab focused on restoring function and independence. Timely therapy helps patients recover safely after surgery, stroke, or hospitalization—facilitating optimal outcomes and smooth discharges. 

— Speech Therapy —

 Speech-language pathologists address swallowing disorders, cognitive-linguistic deficits, and communication challenges post-stroke or illness. In-home therapy supports faster recovery and reduces the need for facility-based rehab. 

— Medical Social Work —

 Our medical social workers assist with psychosocial assessments, care coordination, and community resource linkage. We collaborate closely with your team to address barriers to recovery and ensure safe discharge planning. 

— Home Health Aid —

 Certified home health aides provide essential personal care services, supporting ADLs such as bathing, grooming, and mobility. Their assistance promotes patient safety, independence, and reduces caregiver burden—critical for successful discharges.

Do You Have Other Services Available That Aren't Listed?

We offer all skilled services. This includes therapies, nursing, and home health aides (bathing, dressing, assistance in activities of daily living). However, these services require that our patients have a diagnosis that fulfills the requirements under ACHC and CMS. If you are wondering if you or your loved one meets these requirements, please call our office and speak with our Director of Nursing to get your questions answered.

Who Should I Contact If I Have A Referral?

If you are confident that you or your loved one requires services, contact your primary care physician and let them know you would like services with Integrity Home Health. They will see that the correct documentation is sent to our office so we can start care. You can always reach out to our Intake Team by calling the main office (303) 597-0505 and they will help you along the way.

What If I Don't See My Insurance Provider Listed?

It is most likely the case that we will not be able to take you or your loved one under our care. You are always welcome to call our front desk (303) 597-0505 to see if we have not updated our insurance list and if yours is acceptable.

Can I Request My Own Healthcare Nurse Or Aide?

We staff clinicians according to the area of coverage, availability, and skill set. However, we always respect requests of the patient as much as we are able to do so.

Do You Service All Of Colorado Or Just The Denver Metro Area?

We service Adams, Arapahoe, Broomfield, Denver, Douglas, and Jefferson counties.

How Do You Bill Your Patients And/Or Customers?

We always bill your insurance company first and in the rare cases that a co-pay or deductible is due, you will receive a bill via mail/email according to your preferences which you can then pay by mail, phone, or online.
